О мозге человека. XX век и его последняя декада в науке о мозге человека

Первый прорыв

Еще совсем недавно - каких-нибудь три десятка лет тому назад - в тогда наиболее результативном подходе к изучению мозговой организации мыслительной деятельности человека нас было очень немного (Бехтерева, 1971). Как наиболее результативное методически мы имеем в виду изучение различных физиологических показателей мозговой нейродинамики при психологических тестах в условиях прямого и особенно множественного контакта с мозгом человека. В этом направлении позднее появляются исследования Т. Z. Halgren et al. (1978), G. A. Ojemann (Fried Fetz, Ojemann 1981), G. A. Ojemann et al. (1988), Th. H. Schwartz et al. (1996). Прямой контакт с мозгом осуществлялся при клиническом применении имплантированных в мозг электродов и в операционной. Медицинские, этические и научные аспекты применения имплантированных электродов полно освещались на конгрессе по электроэнцефалографии в Риме в 1961 году. Мы использовали данную методику с 1961 года после ознакомления с ее лечебно-диагностическими возможностями в 1960

году в Бристоле (Англия), в Берденовском неврологическом институте, в лаборатории W. Grey Walter.

Прямой контакт с мозгом человека при использовании имплантированных электродов был исключительно требовательным. Он применялся только при условии показания больному и осуществлялся в условиях, когда на страже интересов этого больного стояли все прямые и косвенные участники наблюдения. При этом, как бы ни было заманчиво познание внутреннего мира мозга человека, не делалось ни одного сколько-нибудь опасного шага в сторону от интересов данного больного в интересах других больных и науки. И как только для диагностики и лечения какого-то заболевания утверждалась альтернативная неинвазивная техника или техника одномоментная, применение долгосрочных инвазивных методов прекращалось.

Через полтора десятилетия после первого применения имплантированных в мозг больных электродов эта техника в 1960-х годах буквально потребовала изучения мозговой организации психической деятельности как одной из важных форм контроля в формуле «Не навреди!» Дело в том, что лечебное вмешательство на глубоких структурах мозга в отдельных случаях вызывало побочный эффект в форме психических изменений, хотя, как известно, и само применялось для лечения психических заболеваний еще с 1950-х годов (Heath, Hodes, 1952; Heath, 1954).

Исследование мозговой организации мыслительных процессов в условиях прямой регистрации различных физиологических показателей жизнедеятельности мозга при одновременной реализации психологических тестов позволило осуществить первый прорыв в проблеме «Мозг и психика», который явился результатом прямого контакта с мозгом человека. За счет рациональной организации исследований удалось получить подробнейшие сведения о жизни отдельных зон мозга. Накопление данных предоставило материал для суждения об организации и механизмах деятельности мозга. Было получено множество принципиально новых данных о функциях подкорковых образований, о функциональной организации коры, об особенностях системы обеспечения мыслительной деятельности, механизмах самосохранения, надежности и собственной защиты мозга. Опыт этого периода исследований обобщен нами в большом количестве публикаций, в том числе в монографиях (Бехтерева, 1971; 1974; 1980; 1988а; 1994а; Bechtereva, 1978; Бехтерева, Гоголицын, Медведев, Кропотов, 1985; Медведев, Пахомов, 1989; Смирнов, 1976, и др.).

Второй прорыв

Второй прорыв связан с новой возможностью получать практически в каждом исследовании одновременно сведения о состоянии всего мозга, всех его зон. Этот прорыв позволил строить новую функциональную анатомию мозга человека, подтверждать имеющиеся и формировать новые концепции о механизмах работы мозга. Второй прорыв происходит сейчас, на наших глазах, в Декаду, посвященную мозгу человека. После почти одинокого нашего пути с середины 1960-х годов в изучении мозговой организации мышления сегодня в самых разных изданиях публикуются результаты исследования этой проблемы с помощью неинвазивной техники (позитронно-эмиссионной томографии - ПЭТ, функциональной магниторезонансной томографии - fMRI, или MSI, однофотонной эмиссионной компьютеризированной томографии - SPECT, модернизированной электроэнцефалографии - ЭЭГ, магнитоэнцефалографии - MEG). Возможности неинвазивной техники позволили не только избежать ограничений точечного нейрофизиологического контакта с мозгом. Они, по существу, открыли и принципиально новые перспективы изучения мозга, открыли и расширяют прорыв нового знания в области мозговой организации мыслительной деятельности, да и всех других видов деятельности человека.

Количество публикаций растет как снежный ком, тем не менее данные разных исследователей пока еще не всегда легко сопоставимы. Этому есть объяснение, прежде всего, в методических причинах, а также и во внутренних, а именно в динамичности систем мозга, обеспечивающих высшую нервную деятельность человека. Пожалуй, если только сказать, что изменения на ПЭТ при мыслительных процессах удалось обнаружить как в левом, так и в правом полушарии - ив коре, и в глубоких структурах мозга, это окажется справедливым как резюме для большинства опубликованных работ. В подавляющем большинстве исследований с предъявлением сложных психологических тестов изменения преобладают в левом полушарии, хотя роль правого полушария было бы неправомерно недооценивать - оно закономерно вносит свой вклад в реализацию теста. Можно еще, пожалуй, уточнить, что в коре изменения наблюдаются преимущественно в левой височной доле, много данных об изменении в лобных долях. Изменения чаще всего есть и в других структурах, в том числе нередко и в мозжечке.
