Причины невыполнения домашних заданий (заполняется пациентом)




Ниже перечислены причины, на которые чаще всего ссылаются пациенты, когда не выполняют домашние задания. Поскольку темпы выздоровления в значительной степени зависят от вашего старания и объема затраченных вами усилий, крайне важно выяснить, что препятствует выполнению домашних заданий. Всякий раз, когда вам не хочется выполнять задание или когда вы чувствуете желание отложить его выполнение, возьмите данный опросник, прочтите приведенные в нем утверждения и отметьте, какие из этих утверждений отражают ваши мысли и настроение. Если вы согласны с утверждением, поставьте рядом с ним букву «В» («верно»). Если вы не согласны с утверждением, впишите букву «Н» («неверно»).

Схема обследования и терапии, принятая в «Центре когнитивной терапии».

А. Предварительная оценка и диагностика.

1. Полная клиническая оценка: психический статус, история заболевания, история жизни.

2. Таблицы аффективных расстройств и шизофрении (SADS;Spitzer, Endicott).

3. Предварительный диагноз.

4. Клинические шкалы: «Шкала депрессии» Гамильтона, «Шкала тревоги» Гамильтона, «Шкала суицидальных намерений» (SSI).

5. Психометрические тесты: Миннесотский многоаспектный личностный опросник (MMPI),«Шкала тревоги и тревожности» Спилбергера, «Анкета-шкала симптомов» (Hopkins); «Шкала депрессии» Бека.

6. Специальные методики: тест «Я-концепция»; «Шкала дисфункциональных установок»; «Шкала безнадежности»; тест «Аранжировка картины»; методика «Завершение истории».

Б. Назначение пациента к терапевту.

В. Первая терапевтическая сессия (проводится в первые три дня после предварительной оценки).

Г. Частота и продолжительность сессий и общая продолжительная лечения: максимум 20 встреч за 12 недель; продолжительность каждой сессии — 50 мин; встречи происходят сначала дважды в неделю, затем — раз в неделю.

В среднем курс терапии включает 15 сессий: 1-3 недели — по 2 сессии в неделю, 4-12 недели — одна сессия в неделю.

Первая сессия

1. Установить раппорт.

2. Выяснить ожидания пациента в отношении предстоящей терапии.

3. Выявить негативные установки пациента.

4. Определить проблемы, требующие неотложного вмешательства (например, чувство безнадежности, суицидальные желания, выраженное снижение уровня функционирования, тяжелая дисфория).

5. Объяснить пациенту суть когнитивно-поведенческих стратегий; обосновать необходимость выполнения поведенческих поручений и домашних заданий.

6. Поручить пациенту описать свой распорядок дня.

7. Выдать пациенту для чтения брошюру «Как победить депрессию».

8. Расспросить пациента о его реакции на проведенное интервью. Внимание: обычно к концу первой сессии настроение и самочувствие пациента улучшаются; если этого не происходит, терапевт должен выяснить причины.

Вторая сессия

1. Расспросить пациента о том, как повлияла первая сессия на его настроение и самочувствие.

2. Просмотреть заполненную пациентом форму.

3. Выяснить реакцию на прочитанную брошюру.

4. Обсудить трудности, с которыми столкнулся пациент при выполнении домашнего задания.

5. Совместно составить режим дня для пациента.

6. Познакомить пациента с методикой оценки мастерства и удовольствия (как возможность).

7. Определить проблемы, подлежащие обсуждению на следующей сессии.

8. Выяснить реакцию пациента на проведенную сессию.

Третья сессия

1. Определить повестку дня.

2. Совместно проследить динамику состояния и самочувствия пациента с момента первого интервью.

3. Проверить, как пациент справился с домашним заданием.

4. Обсудить реакцию пациента на предыдущую сессию.

5. Обсудить негативные автоматические мысли (как возможность).

6. Разработать новое домашнее задание.

7. Выяснить реакцию пациента на проведенную сессию.

8. Попросить пациента написать к следующей сессии краткий автобиографический очерк.

Четвертая сессия

1. Следовать схеме третьей сессии.

2. Продолжать обучать пациента распознавать неадаптивные когниции (при необходимости используя технику «индуцированных образов» или ролевые игры).

3. Объяснить пациенту, как эти автоматические когниции искажают его восприятие реальности и приводят к развитию депрессии.

4. Выявление автоматических мыслей, особенно тех, что препятствуют выполнению домашних заданий.

Пятая сессия

1. Следовать схеме четвертой сессии.

2. Обсудить выполненные пациентом поведенческие задания.

3. Просмотреть и обсудить записанные пациентом негативные мысли.

4. Продемонстрировать пациенту способы оценки и корректирования когнитивных искажений (автоматических мыслей).

5. Обучить пациента ведению «Протокола дисфункциональных мыслей»; объяснить, как заполнять колонки 4-6.

Сессии 6-8

1. Общая схема та же, что и для пятой сессии.

2. Продолжать устранять психологические барьеры, препятствующие восстановлению преморбидного уровня функционирования.

3. Продолжать выявлять негативные автоматические мысли.

4. Продолжать обучать пациента находить рациональные ответы на автоматические мысли.

5. Разработка новых домашних заданий.

6. Обсуждение концепции депрессогенных базовых убеждений (глава 12).

Сессии 8-12

1. Возложить на пациента бóльшую ответственность за определение повестки дня.

2. Делегировать пациенту ответственность за разработку и выполнение домашних заданий.

3. Выявление и обсуждение базовых убеждений. Проверка валидности убеждений.

Заключительные сессии 13-15 (вплоть до 20)

1. Готовить пациента к завершению терапии.

2. Делать упор на том, что и по завершении терапии пациент должен практиковаться в применении поведенческих и когнитивных техник. Объяснять пациенту, что психотерапия — это обучающий процесс, который продолжается всю жизнь.

3. Обговорить с пациентом, какие проблемы он предвидит в будущем и как он может разрешить их.

Затем терапевт попросил ее еще раз мысленно проделать всю последовательность шагов. Через некоторое время пациентка возобновила занятия аэробикой. Терапевт проинструктировал ее, что в случае возникновения каких-то проблем она должна попытаться решить их на месте, а затем рассказать о них на сессии.

[7] В ответ на это возражение Эллис (личное письмо, 1978) пишет: «РЭТ [рационально-эмотивная терапия] не исключает построения "прочной базы данных" и проверки обоснованности выводов пациента. Хотя такая задача впрямую не ставится перед РЭТ-терапевтом, никто не запрещает ему заняться сбором фактических данных. Во многих случаях я выстроил бы диалог с пациентом, следуя той же модели, из какой исходит ваш терапевт; в других случаях я, вероятно, предпочел бы обсудить с пациентом его самооценку или склонность к катастрофическому восприятию ситуаций — и в ходе этого обсуждения собрал бы ту же самую "базу данных". Я допускаю также, что терапевт может подвести пациента к самоприятию и отказу от катастрофического истолкования действительности с помощью совершенно иной информации, отличной от той, которую собирал ваш терапевт. Рационально-эмотивная модель не регламентирует способы опроса пациента и пути обсуждения проблем».

[8] В тех случаях, когда пациент не в состоянии вычленить наиболее беспокоящие симптомы или более или менее вразумительно обозначить свои проблемы, терапевт может воспользоваться «Шкалой депрессии» (Beck, 1967, 1978). Практически каждый пункт данного опросника может открыть доступ к искаженным или дисфункциональным представлениям пациента.
